Below you will find access to our Office Policies and Procedures (including appointment fees). Be sure to check out the New Patient Forms and HIPAA NPP forms as well. New patients are scheduled for a 2-hour time block initially with 2 follow-up appointments, one hour each. Out-of-area patients should plan on spending 2 consecutive days in town so I may follow up with you the day (or 2 days) after your initial appointment. Please call the office for information about local accommodations.

Initial Appointment (2 hours): $672
Regular Appointment (1 hour): $336
***You are only charged for the time you use with Dr. Morgan. For example, if your appointment only lasts 30 minutes, you will be charged half price.***

For new patients, please book an Initial (2 hour) Appointment. For returning patients, select the Regular Appointment (1 hour) option.
***NEW PATIENTS***: After booking your appointment, you can either fill out the Male or Female patient intake form below, OR you can fill out the intake form online. At the booking confirmation page, simply select “Fill Out Intake Form” if you prefer the online option.
